Safety issue

Ive been lurking for a long time without anything to add but love the shared info and the foxes.
I have a friend who picked up an A grade 12 in the 24xxxx range.
He asked me to look at it because occasionally when he fires the first barrel, the safety clicks to safe.
I took it apart and it all seems to be right inside. Safety spring is tight and the notches that the wheel sit in are not worn.
I think he is hitting it with his had. I dont see anything in the working of the fox gun that could cause this from the inside.
Am I missing something?
